This supportive model utilizes federal Substance Abuse Block Grant (SABG) funding to pair intensive peer mentoring and life-skills development with dedicated transitional housing, justice-system re-entry tracking, and direct pipelines to clinical rehabilitation assets to ensure long-term community lifestyle stabilization.
